To develop our staff and future workforce to support the integration and transformation of our services whilst ensuring we recruit and retain talented individuals.

To continue to align and redesign our hospital, community, social care, primary care, mental health and voluntary/community sector services in order to facilitate our integrated neighbourhood approach.

To develop and support our five primary care neighbourhood hubs and key partners to enable them to delivery new integrated service models in order to improve our health and wellbeing objective through supporting people:

  • to prevent ill-health and to live healthy, independent lives wherever possible
  • to manage any ongoing health conditions more effectively in their own homes and communities
  • to access joined-up services in the most appropriate location

To deliver against the required local/national regulatory frameworks and standards in addition to securing the most effective and efficient use of resources to deliver services that we provide directly or indirectly through our partner organisations.
